Cancer occurs when there is an uncontrolled growth of cells. Typically, colorectal cancers begin from benign growths (polyps) on the inner lining of colon or rectum. Not all polyps will turn into cancer, but adenomatous (cancers that begin in cells that produce mucus and other fluids) polyps may have the potential to grow into colorectal cancer after many years.<\/p><p>More than 95% of colorectal cancers are adenocarcinomas. Very rarely, cancers of colon and rectum are due to carcinoid tumours, gastrointestinal stromal tumours (GIST), lymphomas or sarcomas. This writeup refers to colorectal adenocarcinomas.<\/p><p><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-ad2d561 exad-sticky-section-no exad-glass-effect-no elementor-widget elementor-widget-heading\" data-id=\"ad2d561\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Overview of Colorectal (aka Colon) Cancer in Singapore<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-315f48b exad-sticky-section-no exad-glass-effect-no elementor-widget elementor-widget-text-editor\" data-id=\"315f48b\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Colorectal cancer is very prevalent in Singaporeans. In fact, it is the most common cancer in Singaporean men and the 2<sup>nd<\/sup>\u00a0most common cancer in Singaporean women<sup>1<\/sup>. Furthemore, Singaporean men have the highest incidence of colorectal cancers within Southeast Asia, with incidence rates as high as western countries<sup>2<\/sup>.<\/p><p>Every day, 3 Singaporean men and 2 Singaporean women are diagnosed with colorectal cancers<sup>1<\/sup>. Of great concern is the increase in the global incidence trend of colorectal cancers in adults younger than 50 years old. The National Cancer Institute\u2019s SEER Registry (USA) showed the incidence of colorectal cancer is increasing at 2% per year<sup>3<\/sup>. Usually, many changes in DNA over time are required to cause cells to grow out of control.<\/p><p>Most of these DNA changes are sporadic and poorly understood. Certain modifiable risk factors increase the chance of acquiring these DNA changes. They include:<\/p><ul><li>A diet high in processed meat and low in fibre<\/li><li>Excess alcohol consumption<\/li><li>Smoking<\/li><li>Obesity<\/li><li>Lack of physical exercise<\/li><li>Type 2 Diabetes Mellitus<\/li><\/ul><p>There are risk factors that cannot be controlled including:<\/p><ul><li>Older age<\/li><li>Male<\/li><li>Personal or family history of colorectal polyps<\/li><li>Personal or family history of colorectal cancers<\/li><li>Personal history of inflammatory bowel disease<\/li><li>Inherited genetic syndromes linked to colorectal cancers eg: Lynch syndrome<\/li><li>Familial adenomatous polyposis (FAP). Hence, screening for polyps and cancers remains the single most effective strategy in decreasing the mortality from colorectal cancers.<br \/><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-fe89e40 exad-sticky-section-no exad-glass-effect-no elementor-widget elementor-widget-heading\" data-id=\"fe89e40\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Screening for Colorectal Cancer<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-c590853 exad-sticky-section-no exad-glass-effect-no elementor-widget elementor-widget-text-editor\" data-id=\"c590853\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Screening for colorectal cancer means testing healthy, at-risk people, who have no symptoms. The idea is to identify polyps and cancer at the early stage so that intervention bears the highest chance of cure. Unfortunately, in Singapore, only about 2 in 5 individuals who are 50 to 69 years of age go for colorectal cancer screening at the recommended frequency.<\/p><p>There are various screening methods developed for colorectal cancer, with each test having its own pros and cons.<\/p><p><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e099476 exad-sticky-section-no exad-glass-effect-no elementor-widget elementor-widget-heading\" data-id=\"e099476\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">1. Stool testing for traces of blood<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-c6da764 elementor-widget__width-initial exad-sticky-section-no exad-glass-effect-no elementor-widget elementor-widget-text-editor\" data-id=\"c6da764\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>The Singapore Cancer Society has rolled out annual complimentary Faecal Immunochemical Test (FIT) kits to all Singaporeans and permanent residents aged above 50.<sup>5<\/sup>\u00a0FIT is cheap, simple, non-invasive and can be self-administered at home without needing any dietary modification. However, it needs to be done annually and any abnormal results will need to be followed up with colonoscopy to exclude polyps or cancers.<\/p><p><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-c78ee3f exad-sticky-section-no exad-glass-effect-no elementor-widget elementor-widget-heading\" data-id=\"c78ee3f\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">2. Visual inspection of colon<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-cf92dc1 elementor-widget__width-initial exad-sticky-section-no exad-glass-effect-no elementor-widget elementor-widget-text-editor\" data-id=\"cf92dc1\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>A proper visual inspection of the colon will require prior bowel emptying using laxatives to clean the colon.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-1e3f8ef exad-sticky-section-no exad-glass-effect-no elementor-widget elementor-widget-heading\" data-id=\"1e3f8ef\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">3. Colonoscopy<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e1366bc elementor-widget__width-initial exad-sticky-section-no exad-glass-effect-no elementor-widget elementor-widget-text-editor\" data-id=\"e1366bc\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-902 alignright\" src=\"http:\/\/tcc.sg\/wp-content\/uploads\/2024\/01\/Cystoscopy-cancer-test-1-300x171.webp\" alt=\"\" width=\"300\" height=\"171\" srcset=\"https:\/\/tcc.sg\/wp-content\/uploads\/2024\/01\/Cystoscopy-cancer-test-1-300x171.webp 300w, https:\/\/tcc.sg\/wp-content\/uploads\/2024\/01\/Cystoscopy-cancer-test-1.webp 700w\" sizes=\"(max-width: 300px) 100vw, 300px\" \/><br \/>A colonoscope (flexible tube with light source at the tip) is inserted by a doctor through the anus along the whole length of the colon to look for polyps or cancers. Any abnormal spot can be sampled using a small instrument passed through the colonoscope. Light sedation is usually administered prior to the procedure to keep you comfortable.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-eef8bb2 exad-sticky-section-no exad-glass-effect-no elementor-widget elementor-widget-heading\" data-id=\"eef8bb2\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">4. Flexible sigmoidoscopy<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-cef8330 elementor-widget__width-initial exad-sticky-section-no exad-glass-effect-no elementor-widget elementor-widget-text-editor\" data-id=\"cef8330\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>A sigmoidoscope is used to examine the rectum and sigmoid colon and allows sampling of any suspicious lesion. Only the last 60cm of the rectum\/colon is visualised.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-83bf6f6 exad-sticky-section-no exad-glass-effect-no elementor-widget elementor-widget-heading\" data-id=\"83bf6f6\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">5. CT colonography (virtual colonography)<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-ec010ae elementor-widget__width-initial exad-sticky-section-no exad-glass-effect-no elementor-widget elementor-widget-text-editor\" data-id=\"ec010ae\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>A CT machine rotates around the body and takes many images of the abdomen. Special computer software then recreates 3-dimensional images of the inside of the colon and rectum, which allows the doctor to look for polyps and cancer. However, any suspicious lesions cannot be sampled and patients with abnormal results will need to undergo a formal colonoscopy.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-3c722bd exad-sticky-section-no exad-glass-effect-no elementor-widget elementor-widget-heading\" data-id=\"3c722bd\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">When You Should Go For a Screening<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-7556f55 elementor-widget__width-initial exad-sticky-section-no exad-glass-effect-no elementor-widget elementor-widget-text-editor\" data-id=\"7556f55\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>For average-risk individuals, most guidelines recommend colorectal\u00a0<a href=\"https:\/\/tcc.sg\/vi\/services\/screening\/cancer\/\">cancer screening<\/a> to start from 50 years onwards. In 2018, the American Cancer Society lowered the age to start colorectal screening to 45 years old.
